Appointments dates will be scheduled once we have received authorizations from your medical and dental insurance plans.  Once we have received the necessary authorizations we will contact you to schedule your appointment.  Please let us know if you have any changes in your contact numbers.  Authorizations have expirations.  If you do not schedule prior to this dates we will be required to start the process over.

Your arrival time will be given a couple days prior to your appointment.  This time will be scheduled by our doctor based on the patients health history, age, medical conditions and proposed length of treatment (if available).

It is mandatory that patients do not eat or drink after 12:00 midnight prior to the appointment. We understand that it is difficult to keep your child from eating or drinking, but it is for their safety that you strictly adhere to this instruction. We ask that you not brush their teeth so they will not accidently have a sip of water.  The patient will be rescheduled if there is any question that the patient may have had anything to eat or drink.

Parent/ Guardian photo ID.

Change of clothes in case of accident.

Guardianship paperwork (if required).

Headphones for your child if you bring an electronic device for them watch or play.

The entire process, from registration to discharge, can take up to 4-6 hours. Each child's dental treatment will vary with severity and can cause our schedule to back up.  Please be assured that our doctors will spend the necessary amount of time to do quality and safe treatment on each child.

If your child has any medical health conditions  (including but not limited to heart murmur, down syndrome, thyroid, iron deficiencies', obesity) our anesthesiologist may require a medical clearance form from your primary care physician.  We would be happy to fax or email this form to your physician.  But we must have this clearance prior to scheduling the appointment.

It is required by law that the child is brought to the appointment by the "Legal" guardian.  If you have a grandchild or foster child in your care, we require a court order, or proof of guardianship.

Free parking is available at the back of our medical complex.  Please do not park on the street.  Those spaces are time limited and can result in a parking ticket if you exceed the wait time.  CSC will not be responsible for these tickets.

No food or drinks allowed

There are procedures that may not be covered by your insurance (such as white crowns for the front teeth,  and sealants on baby teeth).  These fees and any deductible or copay will be collected the day of treatment. We accept credit card, Care Credit or cash.  No personal checks. Please call our office for the cost.

Because of the children who are waiting to be seen can not have anything to eat or drink, we ask that you do not bring and food or drinks into the surgery center.

Please dress your child in a short sleeve shirt. (No footed pajamas or one piece outfits)  A pull up or diaper if they are prone to bed wetting.  Please remove all jewelry and nail polish.

Due to Covid 19 we are only allowing one parent to accompany the patient to the appointment. Other visitors will be required to wait outside of the office building.  Both patient and parent are required to wear a face mask while in the dental center.
